Know your PAN Card Number
To get your lost PAN Card back, you first have to know your old PAN number. Do you remember that?

1. The way to go about retrieving your lost PAN Card is firstly paying a visit to your nearest Police Station and lodging a complaint stating that your PAN card is lost or missing.

2. Thereafter, write a letter addressed to the Income Tax Department requesting it to issue a duplicate PAN card.

PAN Lost: New PAN Card Application Form

3. Take a printout of the form. Put in all required details submit it along with identity proof, address proof and complaint copy to the PAN Centre/NSDL TIN Facilitation Centre nearest to your present location.

PAN Card Lost : Steps After You've Filled the Form
1. Make payment as you complete filling the form. On successful confirmation of the payment, all you have to do now is to take a printout of the acknowledgment receipt as it appears on the screen. Send it to the said address within 15 days of online application:
“National Securities Depository Limited,

2. The said receipt should be accompanied by the (1) FIR/ complaint copy; (2) a self-attested passport size photo; (3) valid address proof; (4) valid self-attested identity proof; (5) PAN proof (the printout of the receipt available at will suffice)

PAN Lost : Important Points
1. Ensure that you write “APPLICATION FOR PAN CHANGE REQUEST- Acknowledgment Number” on the envelope which you are sending to NSDL.
